Chronic Disease Management Plan
People with a ‘chronic and complex medical need’ may be able to access Medicare benefits to cover a total of 5 Allied Health sessions that help manage their condition. This is called a Chronic Disease Management plan. It was previously known as the Enhanced Primary Care Plan. Your child may be eligible for this plan if they have a chronic medical condition of greater than 6 months or have complex needs that require intervention from more than one health professional. You will need to discuss access to this initiative with your GP.
